| Claim Procedure |

Complete and return the claim form to Blue Cross within 30 days of the expiry of the policy. |
|
Submit satisfactory proof and complete supporting documentation such as reports issued from hospitals, physicians, police, airlines and/or other responsible authorities togetther with the claim form. |
